Flipchart

The Counterweight Flipchart is a handy reference tool which includes our screening and treatment pathways, the benefit of 10% weight loss, a weight loss ready reckoner and appointment planner. Flipcharts are available for all staff actively participating in the programme.
 

Nurse training materials

One training manual is supplied for each trained participant with up to 20 manuals for 10 practices.
 

Patient education materials

A series of 19 patient education booklets and leaflets is provided. Each participating practice is given a year’s supply, this is estimated as 50 sets, with double quantities of the screening booklets given.

A self-assessment tool for patients to determine if they are ready to start a weight loss programme.

Helps to establish fundamental strategies enabling successful permanent lifestyle changes.

Daily Living Diary
A tool to record a patient's food intake and activity levels. Enables them to understand their behaviours and emotions around eating and physical activity..

Managing Difficult Times
Enables patients to plan ahead for times when they lapse. Helps with identification of high-risk situations and choice of 'safe' coping strategies.

Healthy Eating Quiz
A quick food frequency questionnaire used to identify areas of imbalance around fat and carbohydrate in a patient's eating plan.

Goals to Help you Lose Weight and Eat Healthily
A goal-centred approach to decreasing energy intake and increasing activity levels using SMART goals.

My Personal Weight Loss Plan
A prescribed calorie-controlled approach to decreasing energy intake tailored to suit an individual’s requirements.

The Balance of Good Health
A pictorial representation of the proportion of each food group giving a week’s food intake for a balanced diet.
 

Getting The Balance Right
Gives information on the food groups represented in The Balance of Good Health and highlights the importance of lowfat eating.

Snack Attack
Healthy savoury and sweet snack ideas for foods giving less than 120kcal. Used by itself or in conjunction with the booklet My Personal Weight Loss Plan.

Physical Activity
Helps patients to increase their daily activity level with an emphasis on walking for health.


Confused About Food Labels?
Gives guidance on reading and understanding common food manufacturers’ claims along with information on the meaning of food labels.


Shopping, Cooking and Eating Out
Gives ideas to help patients stay with their eating plans during different and possibly difficult situations.

Eating Behaviour
Used to explore internal and external triggers to behaviours around eating that may result in weight gain.
 

 

Maintaining Change
Looks at the roles of positive and negative thinking in a successful change process and summarises the characteristics common to people who have successfully maintained long-term changes.

Used for patients who require antiobesity medication.

Advice for Patients taking Sibutramine
Used for patients who require anti-obesity medication.
