National Digital Type 2 Diabetes Remission Programme
The NHS Scotland Digital Diabetes Remission Programme is an innovative 12-month online initiative designed to help people with Type 2 Diabetes achieve remission through structured weight-loss and lifestyle support.
Delivered by experienced behaviour change coaches, including Dietitians, the programme combines clinically proven dietary stages with ongoing virtual guidance. This approach makes care accessible, effective, and future-focused, supporting Scotland’s commitment to innovative healthcare solutions.
The programme is delivered across NHS Scotland by the Centre for Sustainable Delivery (CfSD) through its Accelerated National Innovation Adoption (ANIA) team at NHS Golden Jubilee, in partnership with Counterweight.
Who is it for?
If you have recently been diagnosed with Type 2 Diabetes and want to improve your health and lifestyle, this programme can help you achieve your goals and remission. You may be eligible if you:
- Have been diagnosed with Type 2 Diabetes in the last 6 years
- Have a BMI of over 27 for White ethnic groups, or over 25 for Black, Asian, and other ethnic groups

How does the programme work?
Each phase of our medically-supervised programme is designed to support participants in building sustainable long-term behavioural change, weight loss and weight maintenance. The programme is divided into three stages:

As part of the programme, participants receive free equipment to monitor blood pressure, weight and blood glucose, uploading results weekly via the app. Virtual coaching sessions are delivered by a range of staff, including dietitians, in order to provide personalised support throughout.
